I was in Naval Aviation when the F4, F14, A3D, A4, A7, and even the RF8 were in active service.
The A3D had the main exit hatch on the bottom of the aircraft. In the event of the aircraft making an emergency landing on water ('ditching') the crew were in a bad spot. I'm sure there were alternate exits but I don't recall their location. At any rate that was the reason for the "A3D='all three dead'" nickname.
